Downloadable Intake Forms

If you have scheduled an appointment with Windsong, look for links in the welcome appointment email and fill out your Health History and Consent Forms online through the Acusimple Patient portal. You should have received an invitation to set up a user Id and password, allowing you to schedule appointments, view patient's documents, and fill out your forms. If, for some reason, you are unable to use the portal, the intake forms are available here to download, fill out, and bring to the office for your initial appointment.
